employee asks mgr; is the procedure this way or that way (legit question, but mgr doesn't know, feels embarressed cause its in front of others)

mgr then asks employee; do you know code a/b/c/...finds weakness...exploits weakness...embarrasses employee

..how it is said is just as important as what it is said...perception sometimes outweighs reality...maybe because we are emotional creatures by nature..

Or when machines fail due to lack of proper maintenance. Employee tries to bring the issue to mangements attention so that the problem can be fixed, management calls machine errors, operator errors when employee operates machine. When mgmnt operates machine, problem is "machine error".

Or, when employee gives "too much info." in incident reports in an attempt to give as much information as possible so that problems can be fixed, the info. can make the problem look like the employees fault.
