More like chemicals in brain go hay wire and feel so hurt that nothink feels worth while feels like the person that was going to be offspring's mother did not want child with , so child never born and heart never got to be born so heart got broken in two and she never cared and through her half of heart away ! I still keep the half of heat that I had as a tattoo on my body half a heart and tryed to make a hole heart half on each wist so I would not hurt a female human same way to remind my self what it feels like so I don't do it like what was done to me to another !

Breakdown of a Broken Heart
Broken heart syndrome, also called stress-
induced cardiomyopathy or takotsubo
cardiomyopathy, can strike even if you’re
healthy. (Tako tsubo, by the way, are octopus
traps that resemble the pot-like shape of the
stricken heart.)
Women are more likely than men to
experience the sudden, intense chest pain —
the reaction to a surge of stress hormones —
that can be caused by an emotionally
stressful event. It could be the death of a
loved one or even a divorce, breakup or
physical separation, betrayal or romantic
rejection. It could even happen after a good
shock (like winning the lottery.)
Broken heart syndrome may be misdiagnosed
as a heart attack because the symptoms and
test results are similar. In fact, tests show
dramatic changes in rhythm and blood
substances that are typical of a heart attack.
But unlike a heart attack, there’s no evidence
of blocked heart arteries in broken heart
syndrome.
In broken heart syndrome, a part of your
heart temporarily enlarges and doesn’t pump
well, while the rest of your heart functions
normally or with even more forceful
contractions. Researchers are just starting to
learn the causes, and how to diagnose and
treat it.
The bad news: Broken heart syndrome can
lead to severe, short-term heart muscle
failure.
The good news: Broken heart syndrome is
usually treatable. Most people who
experience it make a full recovery within
weeks, and they’re at low risk for it happening
again (although in rare cases in can be fatal).""

Why yes it does...
Is it a metaphor,yes it is but when you are going through does it really matter.
When you feel your heart is breaking at the time it doesn't matter it hurts like hell all the same.

is it a broken heart or a broken head ? our brain controls our feelings sending messages all over our bodies so is it a broken heart or broken brain.
some think the heart is the center of the body but the brain controlls the whole body.
example
have you every cut your finger but never noticed it untill you looked at your hand, your eyes saw it transferd it to your brain and then the relisation that your finger is cut

Well, that example ain't exactly correct, cos it would mean you could sever or break a limb and not notice until you'd see your leg laying on the ground, scratch your head "How the heck did that get there?" LOL
But yeah, broken heart has a lot to do with the brain, hormones etc etc

Yes. It is only a metaphor.

While your brain may stress your heart to the point it cannot operate properly, your brain is the faulty organ. Semantics? YOU DECIDE!

Not to say you can't be bummed out by losing a companion. But if that loss makes you die you have terrible coping skills.

And that whole "a broken heart bleeds tears" jazz....lose it.
