Another thing, as a non-native speaker of English it might be helpful to know and understand the connotation of a word in English.
If you think it is respectful and everyone perceives it as unpleasant/too familiar/patronizing or whatever, well ... then you're seriously miscommunicating.
That was the reason why I, a non-native speaker myself, asked the question.
By the way, if you mean to be respectful, why use these angry emoticons all the time?

When you don't know the culture of someone; it's best not to address someone in the familiar state like dear. Dear is a familiar term and if you don't know the person; you shouldn't address them as it. You may think it respectful but other cultures don't agree. Best thing is simply don't address them in any way. Just say Hi.

I've noticed it's usually used by older men when talking to a younger woman, or by men from other countries, like someone else said, usually scammers. I don't care for the term, I think it sounds condescending and overly familiar, it's better to just say hi, especially if you don't know the other person.
